Report an Environmental Health Concern
Have you noticed things like poor food handling, dirty facilities, or other sanitation concerns at a local establishment? District Health Department #10 investigates reports of unsanitary conditions in the following types of facilities:
- Food Service Establishments (Restaurants, Coffee Shops, etc.)
- Campgrounds
- Public Pools
- Tanning Salons
- Body Art Facilites
If you notice a sanitation issue at any of these facilities within the DHD#10 district that does not involve a specific illness, please complete the complaint form below and submit it to any DHD#10 office. Your report helps us take timely action to protect public health.
