Community Connections is a free program providing support to navigate community resources for adults, children, and families.

Services are provided by a certified Community Health Worker who will meet with you in your home or at another location that is convenient to you. They can help you navigate things like transportation resources, health insurance, utility bills, food resources, MiBridges, and more. For more information, to make a referral, or to view program data, please visit our website. Individuals can self consent or legal guardians can consent to a voluntary referral.
