The Children’s Special Health Care Services Program is a program that helps individuals (0-26 years old) that have special needs and health concerns to improve their health and enhance their quality of life. This program can help cover financial costs related to chronic conditions, provide support and care plans, and connect you with the latest resources.

This program will help you and your family:

  • Receive the broadest possible range of appropriate medical care, health education, and supports.
  • Access services and supports that are family centered, culturally competent, community-based and coordinated manner.
  • Help parents and professionals work together.
  • Reduce common barriers individuals with special health care needs experience when accessing services.
