WISEWOMAN (Well-Integrated Screening and Evaluation for Women Across the Nation) helps participants understand their cardiovascular disease (CVD) risk factors and make healthy lifestyle choices with a focus on nutrition, physical activity, and smoking cessation. Enrolled participants undergo a free health assessment and risk reduction counseling with a trained Health Coach. The health assessment includes a full lipid panel (total cholesterol, LDL, HDL, and triglycerides), A1c screening, blood pressure check, and weight check. Participants are also given the opportunity to decide if they would like to set a small lifestyle goal and continue free health coaching with the Health Coach. Those who are eligible for the WISEWOMAN program include women, transgender, or gender nonconforming individuals who are:
- Uninsured or underinsured;
- At or below 250% of the federal poverty guidelines; and
- 35 to 64 years of age.
