Adolescent Health/Wellness Centers have a Nurse Practitioner (NP) or Physician’s Assistant (PA) on staff along with a licensed Mental Health Clinician and a Medical Assistant/Clerk. The following Adolescent Health/Wellness Center’s provide care to anyone age 5-21 who are enrolled in Chippewa Hills School District. Services are provided at low or no-cost.
Chippewa Hills Wellness Center
Serves students age 5-21 enrolled in Chippewa Hills Public Schools.
Primary Care Services: Treatment for sickness and injuries, check ups, sports physicals, immunizations, STD testing & treatment, pregnancy testing & counseling, health education programming.
Mental Health Services: Mental health assessment, individual and family counseling, grief counseling, trauma counseling, and support groups.
- The Chippewa Hills Wellness Center works on a sliding fee scale that promotes access to care to ALL CLIENTS.
- The Sliding Fee Scale assures that no client is denied services based on their inability to pay.
- The discounts in the Sliding Fee Program are based on family size and income.
- The Chippewa Hills Wellness Center accepts ALL insurances including Medicaid.
