Adolescent Health/Wellness Centers have a Nurse Practitioner (NP) or Physician’s Assistant (PA) on staff along with a licensed Mental Health Clinician and a Medical Assistant/Clerk. The following Adolescent Health/Wellness Center’s provide care to anyone age 10-21 who resides or goes to school in Wexford County. Services are provided at low or no-cost.
Wexford Adolescent Wellness Center
Primary Care Services: Treatment for sickness and injuries, check ups, sports physicals, immunizations, STD testing & treatment, pregnancy testing & counseling, health education programming.
Mental Health Services: Mental health assessment, individual and family counseling, grief counseling, trauma counseling, and support groups.
No one will be denied access to services due to inability to pay; and there is a discounted/sliding fee schedule based on family size and income.
School Wellness Programs Grades PK – 12
School Wellness Programs have a Nurse and a licensed Mental Health Clinician on staff. The following Adolescent Health/Wellness Center’s provide care to students in grades K-12 at Manton Schools or Mesick Consolidated Schools. Services are provided at low or no-cost.
