Adolescent Health Centers

The Adolescent Health Center Program services are aimed at achieving the best possible physical, intellectual, and emotional status of adolescents by providing services that are high quality, accessible, and acceptable to youth. Included in the Adolescent Health Program are three models of service delivery – Clinical Adolescent Health/Wellness Health Centers, School Wellness Programs, and Behavioral Health Services models (Expanding, Enhancing Emotional Health).

District Health Department #10 currently supports 5 Adolescent Health Centers, 2  School Wellness Programs, and 3 Expanding, Enhancing Emotional  Health (E3) Mental Health Programs within our 10-county jurisdiction that offer services for adolescents.

All of our centers provide care to anyone age 10-21 who attends school or resides in the respective county and their infants/young children. Services can be billed to health insurance if applicable, and if the client has no insurance, the cost of services is on a sliding scale based on income.

Adolescent Health/Wellness Centers

Adolescent Health/Wellness Centers have a Nurse Practitioner (NP) or Physician’s Assistant (PA) on staff along with a licensed Mental Health Clinician and a Medical Assistant/Clerk. The following  Adolescent Health/Wellness Center’s provide care to anyone age 10-21 who attends school or resides in the respective county and their infants/young children. Services are provided at low or no-cost.

Expanding, Enhancing Emotional Health (E3) Mental Health Program

Below you will find a list of the school-based wellness centers that offer mental health care only. Services are provided by a licensed Mental Health Clinician and include individual, family, and group counseling. Additional specialty services include grief counseling, trauma counseling, and support groups. Treatments plans are developed between the client(s) and the counselor to ensure needs are being met and goals are achieved. If a student has insurance, the insurance is billed for services.  If a student does not have insurance, the student’s family will be referred to the local health department to get assistance in applying for Medicaid.  No student will be denied services based upon insurance status.

Referrals

Adolescent Health Centers accept referrals from school staff (counselors, teachers, administrators) as well as from primary care providers, community mental health, other local providers and parents.

Adolescent Health Center staff does refer students out to other providers when appropriate.
