DHD#10 is hosting a COVID-19 Vaccine Pop-Up Clinic at the following location:
Shelby High School Gym
641 N. State Street, Shelby MI 49455
DATE: Monday, June 7
TIME: 1:00-6:00pm
- Offering Pfizer (Age 12+) and
- Johnson & Johnson (Age 18+)
- Students, families members, and community members welcome
- Vaccine is FREE – Bring driver’s license and insurance card if you have it
- Schedule online (preferred) or walk in. Each person must schedule separately
- Students age 12-15 MUST have parent/legal guardian present
- Students age 16-17 do not need parent/legal guardian present but must bring completed and signed registration and consent forms to appointment.
